Instructions for Creating Your Own Meal Plan

  1. Calculate your calorie LEVEL
  2. Enter your name and contact info into the fields below
  3. Select who your nutritionist is if you have one
  4. Select your LEVEL
  5. Select whether you are going to prepare your own meal, eat a bar, shake or recipe
  6. Select your favorite food in each colored box or select “not in the list” to enter your own
  7. Submit your meal plan for your dietitian nutritionist to review and/or print out a copy of your meal plan
